Interdisciplinary care plan
Our nursing home staff works closely with residents and family members, as well as the residents’ physicians, to develop a multi-disciplinary care plan.

Within 24 hours of admission, your loved one’s healthcare team—which may include a doctor, therapists, pharmacists, registered dietitians, social service workers, activities coordinators and nurses, as well as your own family members—will develop a personalized care plan to fit your loved one’s specific needs. 

The plan may include rehabilitative care, medical care, social support, dietary planning and personal care, as well as Alzheimer’s care if your loved needs it, and it is reviewed regularly to determine if it needs to be revised based on changes in his or her condition.
Quality measure
To ensure we are providing the level of care needed for each of our residents, we monitor many of the same quality measures as the Centers for Medicare and Medicaid Services (CMS). Our data is collected and reviewed at least monthly by our internal Quality Assessment and Assurance Committee, and focuses on the health of each of our residents. Our internal quality programs focus on continual improvement of these quality measures:

• Pain
• Activities of daily living (ADLs)
• Pressure ulcers
• Weight loss
• Antipsychotics
• Physical restraints
• Falls
